                  Award-winning video will help boost patient care

A video to help patients at West Middlesex University Hospital manage their medicines has been selected as the best digital project video by CLAHRC (Collaboration for Leadership in Applied Health Research and Care).

 

It beat 30 other entrants from trusts in North West London and will be screened in the hospital’s Acute Assessment Unit and Medical Day Unit waiting areas, with plans to make it available throughout the hospital and online.

 

The digital video story was produced by West Middlesex’s medicines management project team. It shows footage of the wards, narration by project staff and includes interviews with patients, local GPs and Chief Executive, Dame Jacqueline Docherty.

 

They explain how medications are managed throughout the patients’ journey from referral and admission through to discharge home, and why it’s so important to get this process right. 

 

The judges were impressed by the involvement and enthusiasm of people throughout the healthcare community, allied with the long term benefits demonstrated by the project.

 

Dr Paresh Mistry, Consultant in Acute Medicine and Clinical Lead for the project, explained: “The judges were most impressed by the involvement of a multi-skilled team including our chief executive, and local GPs who had agreed to be interviewed and were very supportive of the work being done. The hospital staff, local GPs, and patients are all part of one community and that shared input is what has helped make the video and wider project such a success.”

 

The five minute video’s content was planned and organised by the project team which included Dr Mistry, Dr Essam Ramhamadany, Assistant Director of Research and Development, Senior Pharmacists: Sandra Chung and Navneet Saggu, and representatives for patients, nursing staff, and Richmond and Hounslow Primary Care Trusts.

 

It was filmed by Registrar Thofique Adamjee, with final editing done by CLAHRC which helps to fund and support research projects to improve patient care.

 

CLAHRC will use the winning video as a tool to share how improvements have been made to West Middlesex’s medicines management plan with other trusts in Northwest London.

The digital project video is the culmination of West Middlesex’s medicines management project which has run between April 2010 and September 2011. The multi disciplinary team has worked together to make innovative changes including:

·        More accurate information about newly admitted patients’ medication

  • A tool that notifies hospital clinicians when there are problems with medicines for newly admitted patients

·        Medication clearly documented and communicated to patients, relatives and GPs

  • An on-site pharmacy based on the AMU ward
  • Easy to understand patient information on manage medicines after leaving hospital
  • More education about medicines management for hospital staff
