This Charter has been developed jointly by Carers UK, London Borough of Hounslow, West Middlesex University Hospital and The Princess Royal Trust for Carers to ensure that carers can continue to play an active role whilst the person they care for is in hospital.
You are a “carer” if you look after a relative, friend or disabled child who needs support to live at home. The support you provide is unpaid.
Recognition Of Your Role
The important role that you play or intend to play in the care of the person should be recognised by healthcare professionals as follows:

Information provided by you about the person should be listened to, recorded and taken into account.

With consent from the cared for person, you should be provided with information, advice and be involved in their care.

You should be included in the discharge process.
As a carer you have a right to:

Request a carer’s assessment giving you the opportunity to discuss the help you need to continue caring. This should take into consideration your work, study and leisure requirements.
